Abstract

In the last few decades, the incidence of caesarean section is increasing in the world, especially in Indonesia. One of the way to treat tissue scar is through biologic and synthetic dressing where nowadays, amnion has been used as biologic dressing frequently. This study was conducted to determine the effect of the use of fresh amniotic membrane on wound incision Caesarean section compared with Caesarean section incision wound covered using regular gauze bandages and fixated with plaster in RS. Dr. Reksodiwiryo Padang. The design of this study is an experimen-tal study with Post test design with control group design. Sampling was done using a formula consecutive sampling two different test samples obtained an average of 72 people for each group. The analysis used include univariate and bivariate analyzes. The average wound healing time the difference was statistically significant (p value <0.05) in the treatment and control groups. There was highly significant difference in the proportion of local infection on day 3 between the treatment and control groups (p value <0.05). There were very significant differences in the proportion of local allergic reactions at day 3, and 5 between the treatment and control group (p <0.05). There are significant differences in terms of the cost of care per day between treatment and control groups (p <0.05). Abstract

Uterine arteriovenous malformations is rare case which cause secondary postpartum hemorrhage. This disor-der can cause massive and suddden vaginal bleeding. Despite of rare case, secondary arteriovenous malforma-tions can occur after a cesarean section. Patients who undergo uterine arteriovenous malformation generally have symptoms of menorrhagia or metrorrhagia after miscarriage, uterine surgery, including cesarean section, and curettage. In severe cases, this can lead to malformations of dyspnoea and heart failure. Management of AVM is by embolization, methotrexate treatment and hysterectomy depending on patient condition. Here reported a case of a patient aged 25 years with a diagnosis of P2H1, AUB ec AVM. Patients got history of caesarean section 6 months ago, and since 5 months ago patients were admitted 8 times for recurrent bleed-ing. Diagnostic examinations found AVM and hysterectomy was done. The AUB diagnosis in patients is less precise, the patient should be diagnosed with secondary postpartum hemorrhage due to uterine arteriovenous malformation acquired. Uterine arteriovenous malformation diagnosis should be considered in patients with secondary postpartum haemorrhage.Keywords: arteriovenous malformation, a former cesarean section, AUB

Abstract

Psoriasis is a residual chronic inflammatory skin disease, characterized by a predominance of pustular eruptions accompanied by systemic symptoms such as fever lasting several days. . Psoriasis in pregnancy can lead to spontaneous abortion, stillbirth and preterm birth. Reported the case of Mrs. YS 38 years old, received from the genital skin section of RS M Djamil Padang with a diagnosis of G3P2A0H2 + pustular psoriasis. Fetomaternal ultrasound was performed, obtained biometrics: BPD: 80mm, FL: 61 mm, HL: 54 mm, AC 271 mm, AFI 10.7 cm, SDAU 2.79, estimated fetal weight 1700-1800 grams. Impression: gravid 31-32 weeks, single live fetus intra uterine. Patients are planned for administration of high doses of corticosteroids. Informed consent to the patient and family about the actions to be performed. Corticosteroid was administered as much as 30 mg long term with a dose of prednisone in tappering off every 2 weeks. The patient was discharged for control to the skin clinic and obstetrics, the patient came 12 days later and was consulted to the obstetrics department for termination of the pregnancy but from the obstetrics department there was no confirmation for termination of the pregnancy. The patient then came 13 days later with 18 hours PRM. Pregnancy termination by cesarean section was performed. a baby girl was born with LBW: 3000 gr, PB: 48 cm, A / S: 8/9, there were no congenital abnormalities such as growth disorders, cleft lip, cataracts, and polycystic kidney disease in infants. Abstract

It remains controversial whether antral follicle count is influenced by hormonal contraception.. This study quan-tified the effect of hormonal contraception on sonographic ovarian reserve markers in 32 users and 32 non-users of hormonal contraception. This was a prospective clinical trial study and has been performed in gynecology Department of M. Djamil Hospital Padang, primary health care March to July 2014. Samples then divided into two groups, group A was users of non hormonal contraception and group B was users of hormonal contraception. On day 2–4 of the menstrual cycle transvaginal sonography was performed and measured repeatedly in the same individual before and after the use of one cycle oral contraceptives (for group B), and on first month and second month (for group A). Statistical analysis to assess significance using the unpaired t test and chi square on SPSS 18.0 for windows. There was no significant association of the antral follicle count between users than non-users of hormonal contraception (p>0,05). There were no significant difference of antral follicle count between users and non- users of hormonal contraception. Abstract

Pregnancy is a dramatic episode of the biological and psychological changes and adaptations of a woman who never experienced it. The incidence of mental disorders in pregnancy was lower than postpartum and outside of pregnancy with 10 - 15% on post partum 10-15% and 2-7% beyond pregnancy. However Ohara reported that 10% of pregnant women was eligible if diagnosed with major and minor depression. There are two phases of pharma- cological management is described in the guidelines panel: the acute phase, symptoms are treated, drug dosage adjusted to prevent the adverse effects and patient given counseling, severe phase, patients were monitored at an effective dose to prevent relapse . In the maintenance phase, patients at risk for relapse is often still treated with medicines. Reported here the case of a female patient aged 31 years diagnosed with G2P1A0H1 term gravid 37- 38 weeks + bipolar affective disorder, current episode manic with psychotic symptoms. Patients have been known to suffer from a mental disorder since 2000 and has been submitted to psychiatry ward for as much as 9 times. Trigger disorder is unknown at this time, the patient suddenly angry for no apparent reason. Patients treated with pharmacotherapy of haloperidol 2x2 mg, roboransia and psychotherapy. Treatments including consideration of the patient to provide pharmacological therapy in a minimun dosage and psychotherapy to effectively reduce teratogenic risk to the fetus. Patients have been known to suffer from a mental disorder since 2000 and has been submitted to psychiatry ward for as much as 9 times. Trigger disorder is unknown at this time, the patient suddenly angry for no apparent reason. Patients treated with pharmacotherapy of haloperidol 2x2 mg, roboransia and psychotherapy. Abstract

Preeclampsia is an important issue in obstetrics because it is still a major cause of maternal mortality compared to bleeding and infection. Similarly, the national prevalence, preeclampsia prevalence in West Sumatra is also constantly increasing. Many factors have been identified as risk factors. This study purposed to identify and analyze risk factors of preeclampsia consist of individual factors. This study was an analytical observational study with case-control design, done in Obstetrics and Gynaecology Department of RSUP DR. M. Djamil Padang on January-December 2013 until samples were completed. Samples consist of 70 preeclampsia patients as the treatment group and 70 healthy maternity patients as the control group in RSUP DR. M. Djamil Padang. The results showed there were significant differences between individual risk factors in the preeclampsia group than the control group.Keywords: Individual Risk Factors, Preeclampsia

Abstract

Using Visual Inspection with Acetic Acid (VIA) as one examination for cervical cancer screening not well-known among health workers in Padang. This study was conducted to know the level of knowledge and attitudes of midwives about cervical cancer screening using Visual Inspection with Acetic Acid (VIA) examination in Primary Health Center in Padang. This was a cross-sectional analytic study and has been performed in the Primary Health Center in Padang from July –August 2014. The selection of the research subjects was performed to a midwife who worked in a gynecological clinic that meets inclusion criteria. We found 48 midwives that met inclusion criteria, guided interviews were conducted with a questionnaire, data recapitulated, and computerized processed using computer software and performed statistical tests. There was a weak correlation between the level of knowledge (X) and attitude (Y) about Visual Inspection with Acetic Acid (VIA) in cervical cancer screening in Padang primary health center. Abstract

The vagina is a membranous muscular tube that connects the vulva and uterus. Congenital or congenital abnormalities in the form of complete or partial absence of the vagina (vaginal agenesis). Patients who experience vaginal agenesis have less frequency, namely 1 in 4000 births, 1 in 4000 to 10,000 births (ACOG). Meanwhile in Dr. Cipto Mangunkusumo Jakarta from 1995 to 1999, an average of 10-12 cases per year. It has been reported the case of a female patient aged 14 years who entered the Gynecology Ward Dr. M. Djamil Padang on January 25, 2014 at 11.00 WIB with a delivery from the RSMJ gyn clinic with a diagnosis of transverse vaginal septum + hematometra + hematotrachelos + hematokolpos planned for a septal incision. After performing the operative action, ± 500 cc of blackish brown liquid was successfully removed.Keywords: Agenesis Vagina, TVS, Tranverse Septum Vagina, Case Report
